Adenomyosis is a common uterine disorder whose mechanisms of development remain insufficiently understood. In recent years, the endometrial–myometrial interface, where key processes of tissue remodeling occur, has received increasing attention. This review aimed to systematically synthesize and critically evaluate current evidence on the causes, features, and pathogenetic role of alterations in the endometrial–myometrial interface in adenomyosis, as well as the mechanisms underlying remodeling in this region. We analyzed papers from eLIBRARY and PubMed databases published between 2016 and 2025 and addressing fibrotic remodeling of the endometrial–myometrial interface; inflammatory, immune, and neuroangiogenic mechanisms of its transformation; and molecular regulation involving microRNAs. Structural and functional changes of the endometrial–myometrial interface in adenomyosis may substantially contribute to impaired female reproductive function. Despite growing interest in the endometrial–myometrial interface and its transformation in adenomyosis, available findings remain inconsistent and require further investigation.
